Breakdown Sheets

Sorry if this is in the wrong forum but I am trying to complete my pre production log and am having trouble completing the breakdown sheets with the software I am using.

Does anyone know where I can aquire a breakdown sheet template as I seem to be able to work better with writing the props etc. with a pen.

I'm not the most naturally organised person in the world, but from what I recall here's what we did for Crooked Features:

For every scene we took a sheet of A4 paper and headed it with the scene number. Then we went through the scene with a fine tooth comb and made a note of every concievable prop that was called for.

Then we used this data on the callsheet. Simple!
